The Puerto Rico boat was docking in the port of Sansouci in
Dominican Republic when the ropes holding
it broke and the vessel crashed into the
quay dragging a loading ramp along with it...

Maybe there was a situation that went out of control,
or a system failed.
There are always problems when something's new.
Sometimes minor,
other times major like this..
